Dear Parent/Guardian;
Please make arrangements to pick up your student’s medication from the school Health Office on or before the last day of school; June 12, 2020. ALL prescription medications must be picked up by the parent/guardian. Medication not picked up will be appropriately discarded. The school cannot store medication in the office over the summer.
Students may pick up their own over-the-counter medications. If you are not comfortable with your child bringing home these medications, please let me know and arrange to pick them up.
Any medications taken during the upcoming school year such as prescription meds, Epinephrine-pens, inhalers and insulin will need to have a signed physician’s order on the “Dispensing Prescription/Non-Prescription Medication” form found on the school website. There are two places you can access this form, either under “enrollment forms” or “Health Office forms”. NEW ORDERS ARE REQUIRED EVERY SCHOOL YEAR. Please bring the completed form to the Health Office. You may also fax this form to school. The fax number is 414-351-7526.
Medication must be brought to school in the original container and properly labeled with the student’s first and last names.
We will NOT accept any medication brought to school in plastic baggies, paper bags, envelopes, etc.
All Individual Health Plans, and Emergency Action Plans must be updated every school year. Please call nurse to schedule an appointment prior to the new school year starting.
